Duties
- Supervise and coordinate daily activities of residents, ensuring adherence to individualized care plans that include assistance with Activities of Daily Living (ADLs), medication administration, and behavioral management.
- Lead and support direct support staff by providing training on assisted living techniques, dementia care, Alzheimer’s care, and working with individuals with disabilities or developmental challenges.
- Monitor resident safety by conducting regular checks, managing emergency situations using first aid skills, and operating EMR (Electronic Medical Records) systems for accurate documentation.
- Assist with meal preparation, light cleaning, and maintaining a clean and safe living environment in compliance with health standards and HIPAA regulations.
- Facilitate communication between residents, families, healthcare providers, and case management teams to coordinate care effectively.
- Manage behavior management strategies and implement conflict resolution techniques to foster a peaceful community atmosphere.
- Drive residents to appointments or community outings as needed while adhering to safety protocols such as heavy lifting procedures and proper use of Hoyer lifts or resident transfer techniques.
Experience
- Prior experience working in assisted living facilities, nursing homes, or home care environments is highly preferred.
- Demonstrated knowledge of medical terminology, medication administration protocols, and hospice or palliative care practices.
- Experience supporting individuals with autism, dementia, Alzheimer’s disease or other cognitive impairments is advantageous.
- Background in social work or case management that emphasizes person-centered care approaches.
- Familiarity with EMR systems for documentation and compliance purposes.
- Valid driver’s license with a clean driving record for transportation duties.
- Ability to perform heavy lifting safely and handle physically demanding tasks such as resident transfers using Hoyer lifts or similar equipment.
