Job Overview
As CBRS/Case Manager, you will play a pivotal role in assisting and coordinating Life Change Associates' new Intensive Outpatient Program. Your expertise will ensure that patients receive comprehensive care tailored to their unique needs, promoting recovery and enhancing their quality of life. You will collaborate with multidisciplinary teams to implement effective rehabilitation strategies while following through with treatment plan objectives at the agency and in the community.
What you’ll do
- Be assigned your own caseload with both Case Management and CBRS, ensuring high-quality patient care and adherence to best practices.
- CBRS providers will provide skills training techniques to patients to help them improve their basic living skills to foster greater independence.
- Case Managers will assist patients with finding community resources (financial, housing, etc...)
- Assist with creating individualized treatment plans and reviewing these plans on a consistent basis.
- Assisting with weekly groups as a co-facilitator.
- Oversee discharge planning processes, ensuring smooth transitions for patients.
- Ensure compliance with HIPAA regulations and maintain accurate medical records for all patients.
- Carrying a crisis phone.
- Job includes weekday services and could include occasional weekend. Providers will set schedule with patients.
Basic qualifications
- Bachelor Degree in a human/health services field
- Familiarity with behavioral health, mental health, skills building techniques
Job Type: Full-time
Pay: $25.00 - $30.00 per hour
Expected hours: 20 – 35 per week
Work Location: In person
