Liberty Village of Princeton is a beautiful retirement community that is looking for an enthusiastic and motivated Admissions Director to coordinate the admissions process in their state-of-the-art skilled nursing facility
Admissions Director Responsibilities:
- Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health and welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patient/resident
- Complete tours, admission process, discharge planning; development and implementation of social care plans and patient/resident assessments as needed
- Refer patient/resident families to appropriate social services agencies when the facility does not provide the services or needs of patient/resident
- Provide information to patient/resident/families as to Medicare/Medicaid, and other financial assistance programs available to the patient/resident
- Complete Social Histories
- Participate as member of interdisciplinary team in preparing care plans
- Document all conditions, attitudes, responses of residents quarterly or as needed
- Serve as liaison between facility, residents, family, responsible parties and outside agencies
- Provide social and psychological support for residents
