What You’ll Do
Oversee the MDS assessment process to ensure compliance with state, federal, and facility standards.
Accurately complete and manage resident assessments and care plans.
Collaborate with interdisciplinary team members to ensure quality, individualized resident care.
Monitor data collection and timely submission of MDS records.
Educate and guide staff on MDS processes and documentation requirements.
Assist with audits, reporting, and regulatory compliance activities.
Qualifications
RAC-CT Certification required.
Current and active RN or LVN license in Texas (unencumbered).
Current CPR certification (or ability to obtain/maintain).
Strong knowledge of nursing/medical practices and long-term care regulations.
Excellent analytical, communication, and organizational skills.
Ability to make independent decisions, manage priorities, and follow through effectively.
Physical ability to lift 60–70 lbs and remain on your feet during shifts.
Positive, team-oriented attitude with strong interpersonal skills.
Must successfully pass a background check, drug screen, and physical (if required).
