Job Purpose/Function: Under the direction of the Program Director, creates and maintains an appropriate linkage system with various community agencies to assist clients in obtaining entitlements and other basic needs including housing, job placement and vocational training, medical services, transportation, education, money management, and life skills. The Services Coordinator supports and enhances the client’s engagement in treatment and also plays an essential role in case management.
Major Duties and Responsibilities:
- Complete and file Authorization Forms for prospective and current residential clients
- Coordinate care with counselors and discharge planner to ensure that authorizations are documented correctly
- Review all initial and con-current authorization forms for accuracy and completeness.
- Tracks waivers through the system until prior authorization is received
- Assist in completion of applications, authorizations and other paperwork that may be necessary to access entitlements or resources
- Assist with care coordination with other community providers
- Facilitate psycho-educational groups when assigned by supervisor
- Attend all program staff meetings to promote care coordination with medical and clinical staff
- Upload all completed authorizations to clients charts
- Ensure that all discharge plans are completed in a timely manner
- Coordinate activities with external agencies such as CSSD to ensure all documentation needed for plan of care is received and documented in a timely manner.
- Attend training sessions, in service seminars, and workshops and staff meetings
- Become familiar with 1115 Waiver requirements and ASAM criteria
- Meet individually with clients to compile accurate information to ensure client is receiving the appropriate time needed to be successful in his recovery process.
- Performs other duties as assigned.
Education and Experience Required:
- CAC or License Eligible- Has obtained a Bachelor’s Degree in a related field such as Case Management, Human Services, Social Work, Mental Health or Behavioral Health Counseling preferred
- Recovery Coach Professional (RCP) designation considered
- Two to three years of experience in the provision of case management services in the substance abuse or mental health field
- Experience navigating the CT system of care for Medicaid Members and uninsured individuals
- Experience creating Wellness Recovery Action Plans (WRAP).
- Experience with Medication Assisted Treatments and practices (MATs)
- Must have a work history that demonstrates the ability to get work accomplished on time
- Must have strong oral and written communications skills along with the interpersonal skills to work effectively at all levels of the organization and its stakeholders
- Cross Cultural Skills and experience with culturally diverse populations
- Bi-Lingual Skills preferred
