Responsibilities
The Care Transition Intake Coordinator Home Health is responsible for receiving and analyzing incoming referrals and assisting with acquiring documentation needed from host hospital for proper patient evaluation.
· Assigns all new referrals, resumptions of care, and patient updates to appropriate CTC for transitioning into LHC agency.
· Ensures all patient benefits are verified by LHC Insurance department and informs CTC and/or LHC agency of any follow up needed.
· Serves as liaison between the host hospital discharge planners, CTC, and accepting LHC agency.
· Communicates patient discharge dates with the appropriate LHC agency to ensure staff availability and scheduling.
· All other duties as assigned.
Education and Experience
- 1 Year experience in home health care
- Current RN or LPN license
- Ability to communicate effectively, work well with others, handle stress, use good judgment, be creative and accept challenge.
- Excellent organizational skills.
- Good understanding of home health issues.
- Knowlege of Medicare, KMA Private insurance.
