What are the responsibilities and job description for the Part Time License Practical Nurse position at LGARPA Corp?
The LGAR Helath and Rehabilitation Center is a 4- Star, 120 bed Skilled Nursing Facility nestled in the hills of Turtle Creek in the eastern suburbs of Allegheny County. We are currently seeking a Part Time Licensed Practical Nurse. to provide care within the scope of the license, standards of care and facility protocols.
Responsibilities:
- Receives and gives intershift report to licensed and nurse aide staff, assures the care plan is adhered to or revised when needed.
- Demonstrates correct medication administration and treatment delivery; educates residents about their medications, recognizes and monitors for side effects:documents according to policy. Implements and evaluates pain management.
- Documents in nurse's notes or other locations in accordance with standards of cre and facility policies; Maintains clinical record in order.
- Provide, supervise and assist with direct resident care; including but not limitied to: call bell response, toileting, feeding, grooming, transrer, ambulation, resotrative services; Implement, evqaluate and document behavior management.
- Complete physical, emotional and psychosocial assessments; Respond to changes in condition; conduct regular rounds.
- Perform resident admissioon, transfer and discharge clinical responsibilities.
- Assures clinical tracking documents are complete, including, but not limitied to I & O, food consumption, blood glucose, weights, neuro checks, skilled and rehab needs, incidentand accidents, pain management, Braden adn other skin assessments, fall risk assessments.
- Obtains specimens using correct technique, as prescribed.
- Complete weekly skin meausrements and document according to facility protocol. Review weekly wound sheets. Complete weekly Falls assessments and document trends and follow up actions. As assigned, provide information to the Director of Nursing who will present it to QA Committee.
- Assure proper count and documentation o fthe controlled drugs.
- Communicate effectively with residents, families, physicians, interdisciplinary team, external governing agencies and external clinical agencies ( such as hospice, diagnostics, emergency services)
