Job Summary:

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct nursing care to clients in accordance with the nursing plan of care, physician orders, and applicable regulations. 

Key Responsibilities:

  • Administer prescribed medications and treatments as ordered by healthcare providers.
  • Monitor patient condition and report significant changes.
  • Document all care provided accurately in the client's record or electronic system.
  • Assist with activities of daily living (ADLs), including hygiene, mobility, and nutrition.
  • Collect samples for lab testing and perform routine diagnostic tests (as allowed by scope).
  • Maintain a clean, safe, and therapeutic environment for patients.
  • Participate in care planning and client education when appropriate.
  • Communicate effectively with clients, families, and interdisciplinary team members.
  • Adhere to all applicable Texas Board of Nursing (BON) rules and standards.

Qualifications:

Education:

  • Graduate of a state-approved Vocational Nursing Program.

Licensure:

  • Must hold a current, valid LVN license issued by the Texas Board of Nursing.

Certification:

  • Current Basic Life Support (BLS) certification from an American Heart Association or Red Cross-approved provider.
  • Additional certifications (e.g., IV Therapy, CPR, First Aid) may be required based on facility needs.

Experience:

  • Previous LVN experience preferred, but new graduates are welcome to apply.
  • Experience in behavioral health, long-term care, or residential/group home settings is a plus.
  • Experience working with adolescent children, preferred 

Skills and Competencies:

  • Knowledge of Texas LVN scope of practice and nursing standards.
  • Strong observation, assessment, and documentation skills.
  • Ability to work independently and in a team environment.
  • Excellent communication and interpersonal skills.
  • High level of professionalism and ethical standards.
  • Ability to train new staff around medication administration
  • Strong verbal, and written communication skills
  • Time Management
  • Working knowledge of Microsoft Office applications (Word, Excel, Outlook) is required to perform documentation, communiation , and data management tasks

Work Conditions:

  • May require lifting, transferring, or repositioning patients.
  • Must be able to stand and walk for extended periods.
  • Shift work, weekends, and holidays may be required.
  • May require being in ratio based on staffing needs to comply with Texas Minimum Standards and Texas Health and Human Services Commission (HHSC)
  • May require transporting clients to and from appointments, activities, or other approved destinations. 
  • A valid driver's license and clean driving record is required
