Responsibilities
- Conduct and complete accurate, timely Minimum Data Set (MDS) assessments in compliance with federal and state regulatory requirements
- Coordinate with case managers, therapists, nursing staff, social services, and other interdisciplinary team members to develop, update, and monitor individualized resident care plans
- Track and document resident progress, update MDS data as conditions change, and ensure resident care plans remain aligned with clinical needs
- Manage timely submission of all required MDS documentation, ensuring regulatory compliance and audit readiness
- Educate and support clinical and ancillary staff on care plans, documentation practices, and resident care coordination
- Monitor outcomes, follow through on care plan interventions, and recommend adjustments as needed
Qualifications
We welcome applications from LVNs and RNs who are detail-oriented, organized, and enjoy a planning/coordination role within skilled nursing care. Ideal candidates will have:
- A current Texas LVN or RN license
- Experience in a skilled nursing facility or long-term care setting
- Previous MDS coordinator or MDS nursing experience strongly preferred
- Strong documentation skills and familiarity with electronic health records (like PointClickCare) or MDS-related software
- Excellent communication skills, with proven ability to work collaboratively across disciplines and support staff training
- The ability to work independently, take ownership of the MDS process, and respond proactively to resident care changes

Compensation is based on your experience level, with a starting rate above typical floor nurse pay to reflect the specialization and consistency of this role.
