Job Summary And Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.

What you will do in this role:

You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.

You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.

You will provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.

You will instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.

You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

What qualifications you will need:

Required: Valid physical therapy license in the state(s) where services are rendered.

Preferred: One (1) year health care experience in any capacity

Required: BLS from an approved agency within 3 months of hire/transfer

Required: Graduate of an accredited program of physical therapy, culminating in a minimum of a bachelor-of-science degree.

Required:

Effective verbal and written English communication skills.

Demonstrated basic to intermediate skills in Microsoft Word, Internet and Intranet navigation.

Highest level of professionalism with the ability to maintain confidentiality.

Good communication skills including ability to handle complaints, objections and resolve conflicts.

Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.

Customer service oriented with the ability to work well under pressure and demonstrates the ability to field intermittent interruptions while providing optimal care to patient.

Deliver service through a team approach that includes all disciplines, team members and patients.

Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.

Strong analytical and problem solving skills.

Ability to work with minimal supervision, take initiative and make independent decisions.

Ability to deal with new tasks without the benefit of written procedures.

Approachable, flexible and adaptable to change.

Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.

Travel may be required in instances where the therapist is asked to work in other EnduraCare locations.
