Job Summary
We are seeking a dynamic and compassionate Registered Nurse to join our healthcare team, delivering exceptional patient care across diverse medical settings. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories using advanced EMR (Electronic Medical Record) systems (PCC)
- Administer medications via IV infusion, injections, or other methods while adhering to aseptic techniques and safety protocols.
- Manage complex care procedures including wound care, catheterization, tracheostomy management, and tube feeding with precision and compassion.
- Perform diagnostic evaluations such as laboratory specimen collection/processing and assist with imaging procedures.
- Provide patient education on health coaching topics including chronic disease management (e.g., diabetes, cardiac), medication administration, and discharge planning.
- Maintain meticulous medical documentation of patient care activities, ensuring compliance with ICD coding standards (ICD-9/ICD-10) for accurate record keeping and billing purposes.
Experience
- Proven experience as a Registered Nurse in acute care settings such as skilled nursing facilities or Long term care.
- Demonstrated expertise working with diverse populations including geriatrics, behavioral health patients, and individuals with developmental disabilities or dementia care needs.
- Hands-on experience with specialized procedures such as tracheostomy care, dialysis support, ostomy care, or assisted living environments.
- Familiarity with EMR/EHR systems like PCC; proficiency in ICD coding (ICD-10), CPT coding for procedures is preferred.
- Familiar with infection control. Enhanced Barrier and Isolation Precautions.
- Prior roles involving home & community care experience or post-acute care settings are advantageous.
