Responsibilities
- Implement individualized habilitation plans that focus on skill development in areas such as communication, socialization, self-care, and community participation
- Assist clients with Activities of Daily Living (ADLs), including grooming, dressing, meal preparation, and personal hygiene
- Support behavioral health needs through positive behavior management strategies and de-escalation techniques
- Facilitate engaging activities tailored to each client’s interests and goals, encouraging independence and social interaction
- Collect and document progress data accurately to monitor growth and inform ongoing care strategies
- Administer medications following prescribed protocols when required and ensure proper documentation
- Drive clients safely to appointments, community outings, or activities as needed while maintaining compliance with all safety regulations
Requirements
- Experience working with people with developmental disabilities, autism, or behavioral health conditions preferred
- Knowledge of special education principles and behavior management techniques
- Strong communication skills to effectively interact with clients, families, and team members
- Ability to assist with ADLs and provide compassionate caregiving in home or community settings
- Valid driver’s license and clean driving record for transportation responsibilities
- Familiarity with data collection methods related to client progress and service documentation
- Experience in social work or home & community care is a plus
- Certification or training in medication administration is desirable but not mandatory; training will be provided if necessary
- Ability to work independently while collaborating as part of a multidisciplinary team
