Job Overview
We are seeking qualified, dedicated and compassionate Private Duty Nurse-PDN (RN or LVN) to join our team of home health care practitioners to provide personalized care to patients in their homes. We currently have open positions for a pediatric patient in Wylie TX 75098. The client attends school from 8:15 A.M. to 3:15 P.M. Monday to Wednesday. The nurse is required to take care of only the client at school and at the house after school. PDN services are long-term hourly nursing care at home for children and adults diagnosed with a chronic illness, injury, or disability. Some driving will be involved. The ideal candidate should be committed to best practices, possess a strong background in patient care, compliance with all applicable laws in the State of Texas. You will be working in the patient’s home. Strong communications skills, compassion, and sincerity is necessary.
This role requires a commitment to delivering high-quality nursing services while ensuring the comfort and safety of patients.
Duties
- Conduct comprehensive patient assessments to determine individual healthcare needs.
- Monitor vital signs and report any changes in patient conditions.
- Administer medications and treatments as prescribed by healthcare providers.
- Administer gastrostomy (G-T) feeding.
- Assess patency and care for G-Tube
- Provide basic life support and emergency care when necessary.
- Collaborate with other healthcare professionals to develop and implement patient care plans.
- Assist patients with daily living activities, ensuring their comfort and dignity.
- Maintain accurate medical records and documentation of patient care.
- Educate patients and their families on health management and disease prevention.
- Ensure compliance with all safety protocols, especially in critical care situations.
Qualifications
- Current nursing license (RN or LPN) with home health experience is a plus.
- Proficiency in performing patient assessments and monitoring vital signs.
- Experience in G-Tube management and tube feeding is highly desirable.
- Knowledge of long-term care practices and patient management strategies.
- Provides care using infection control measures and assures the continuity of care through delivery of quality patient/client care
- Certification in Basic Life Support (BLS) is required.
- Strong communication skills, both verbal and written, for effective interaction with patients, families, and healthcare teams.
- Ability to work independently while demonstrating excellent problem-solving skills.
