The Community Supports Program Lead plays a key leadership role in guiding Community Supports staff and ensuring program operations align with PHG’s mission and values. This role supports staff development, operational oversight, and program performance while promoting a positive, compliant, and collaborative work environment.
Key Responsibilities
Leadership & Team Support
• Lead by example in communication, professionalism, collaboration, and work ethic
• Model PHG values and promote a respectful, inclusive team culture
• Provide daily support, coaching, and guidance to Community Supports team members
Staff Development & Performance
• Conduct regular check-ins and provide feedback to support employee development
• Identify skill gaps and collaborate with the Program Manager on training initiatives
• Monitor performance and address concerns proactively
• Document coaching conversations and escalate concerns to the Program Manager when necessary
Operational Oversight
• Oversee team schedules, workflow, and documentation requirements
• Ensure timely completion of tasks, reports, and training requirements
• Track PTO usage and maintain appropriate staffing coverage
• Support program efficiency and workflow improvements
Policy Compliance
• Maintain understanding of company policies, labor laws, and program requirements
• Ensure team members follow PHG policies and operational procedures
• Partner with Human Resources and the Program Manager on compliance matters
Change Management & Communication
• Support implementation of new programs, processes, or technology initiatives
• Communicate updates clearly and effectively to team members
• Serve as a liaison between leadership and staff during transitions
Onboarding & Employee Support
• Assist with onboarding and orientation of new team members
• Help foster a supportive and engaging environment for new hires
Employee Relations
• Serve as a first point of contact for employee concerns
• Address conflicts professionally and promote respectful communication
• Escalate sensitive or complex issues to HR or the Program Manager as appropriate
Qualifications
Required
• 3–5 years of experience in case management, social services, healthcare, or a related field
• Familiarity with Medi-Cal, CalAIM, and Community Supports programs
• Strong understanding of healthcare systems and community resources
• Excellent communication and interpersonal skills
• Strong organizational skills and attention to detail
• Proficiency with case management platforms and related technology
Preferred
• Experience leading or supervising teams
• Strong empathy and cultural competence
• Experience with needs assessment and care planning
• Service coordination and client advocacy experience
• Motivational interviewing skills
• Strong problem-solving and decision-making abilities
• Ability to collaborate effectively across multidisciplinary teams
