Overview
As a Virtual Nurse you will play a crucial role in enhancing resident care by ensuring thorough and timely admission and discharge assessments are completed. Your contributions will directly impact resident safety, care coordination, and overall healthcare outcomes.
How you Will make a difference
- Conducting Virtual Assessments by completing comprehensive admission and discharge assessments via Care.ai.
- Communicating critical resident information to floor nurses.
- Reviewing initial hospital discharge records and gathering additional medical, psychosocial data from needs assessment, client/family, physicians, and other health care providers.
- Collaborate with multidisciplinary care team to facilitate timely resolution of situations such as resident concerns, need for referrals and discharge barriers for the discharge plan.
- Identify and participate in process improvement initiatives that improve customer experience, enhance workflow, and/or improve the work environment.
- You’ll comply with all of Signature’s rules, regulations, and nursing standards
- You’ll maintain privacy and security of all confidential and protected health information
- Other special projects and duties, as assigned.
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) in state employed
- Minimum of three (3) years of Licensed Practical Nurse (LPN) or Registered Nurse (RN) experience in an Skilled Nursing Care setting
- Highest level of professionalism with the ability to maintain confidentiality.
