FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $19.67 - $25.57 / hour. Pay is dependent on applicant's relevant experience

Summary

Researches and schedules new patients to a primary care provider. Guides patients with proactive outreach, facilitation and coordination of care delivery. Individualizes onboarding experience for new patients referred by specialist, hospital visits and community line.

Responsibilities

  • Answers phone requests to locate and coordinate care for new patients, finding and assigning PCP based on needs and opportunities.
  • Works in-basket pool with requests from specialty for assignment of PCP and hospital discharges.
  • Prepares new patients for first visit, including assisting patients to sign up for MHC, requesting medical records from previous entities, uploading history into EPIC and health maintenance.
  • Communicates with department assigning patient for appropriate time required for visit and appropriate timing of visit.
  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements

  • High school diploma or GED.
  • 2 years of healthcare experience.
