Remote LVN/LPN Nurse Coordintor

What You’ll Do
- Engage with patients and caregivers to ensure therapy adherence
- Assist patients in starting prescribed treatments
- Address therapy-related concerns with empathy and accuracy
- Communicate with charge nurses in nursing homes as needed
- Handle 50–70 inbound and outbound calls daily
- Maintain precise documentation and follow templates
- Participate in audits to ensure compliance and quality
- Active Compact LPN/LVN License Required
- Prior experience working in a high volume setting meeting daily metrics
- Prior experience working in a remote setting
- Professional phone presence and patient-first mindset
- Commitment to 98% accuracy in documentation and patient interactions
