Works under the supervision of the Director of Home Care. Directly supervised by the RN staff to provide nursing care according to established Home Health policies and procedures. Provides skilled nursing care to clients in their place of residence, contributing to the planning, implementation, and evaluation of nursing care to meet the age specific need of clients. Assist with patient teaching, observation and delivery of nursing care.
Education, Work Experience and Licensure:

Education, Work Experience and Licensure:
- Graduate of an accredited school of practical nursing.
- Holds a current nursing license in good standing for Ohio and West Virginia.
- Minimum of one year nursing experience required.
- Six months to one year home health experience desired.
- Previous experience preferred.
- Valid driver’s license, proof of auto liability insurance and clean driving record. A clean driving record includes, but is not limited to the following: no OVI, no reckless operation, and no failure of physical control on driving record. If such violations should occur after employment, employee may not continue working in the Home Care department. Citations will be reviewed on a case by case basis.
- No criminal record as validated by criminal background check, including fingerprinting.
