The Social Service Director (SSD) is responsible for meeting the psychosocial needs of our residents. SSD tasks include:
- Completing Social Service assessments i.e., MDS, psychosocial and trauma assessments
- Developing a person-centered plan of care with residents, their families, and an interdisciplinary healthcare team
- Encouraging residents to participate in their plan of care and set realistic, attainable goals
- Reporting concerns to the Administrator
- Managing grievances
- Provide support to residents and families coping with aging related illness, grief, and adjustment to new residence.
- Discharge planning and coordination of community services
- Assisting residents with Advance Directive and legal documentation and maintaining appropriate documentation for HCP, Guardians and Rogers reviews etc.
- Use resources to effectively assist residents to attain or maintain the highest practicable physical, mental, and psychosocial well-being for each resident.
- Offer daily contact with residents, providing opportunities to vocalize concerns and provide reassurance that staff at the home cares about, and is interested, in their welfare
- Long term care nursing home experience preferred
- Social Service or related degree
