LVN- Hospice

Lenity Hospice is currently looking to add License Vocational Nurses (LVN) to its great team of clinicians. This position will be require to work on-call and available on weekends if needed. Applicant must be willing to travel throughout the Central Valley. We are looking for someone who is family/patient focused and compassionate about Hospice.

Job Description

Responsibilities:

• Provide direct patient care.
• Assess and provide patient, family/caregiver education to diagnosis.
• Provide appropriate pain/symptom management.
• Participates in coordination of hospice services.
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Qualifications:

• 1 year clinical experience. Hospice Preferred.
• Excellent oral and written communication skills.
• Ability to interact effectively with all staff, patients, families, and members of the health team.
• Effective problem-solving and analytic skills.
• Computer skills as the position require documentation on hospice software program.
• California LVN/LPN license, CPR, and auto insurance.
