Duties

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote independence and dignity.
• Monitor patient health by observing vital signs and overall condition; report any concerns promptly to healthcare professionals.
• Support medication administration following prescribed protocols while adhering to HIPAA privacy standards.
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
• Maintain a clean and safe environment through regular cleaning duties and infection control practices.
• Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfer and mobility assistance.
• Document care plans, patient observations, and incident reports accurately in compliance with healthcare regulations.
• Provide emotional support and companionship, fostering social engagement and mental well-being for residents or clients.

Requirements

• Proven experience in home & community care, assisted living, nursing homes, or working with individuals with developmental disabilities or dementia.
• Knowledge of patient observation techniques, medication administration, vital signs monitoring, and emergency response procedures including CPR and First Aid certification.
• Familiarity with specialized care such as Alzheimer's care, memory care, hospice care, autism support, and working with people with disabilities.
• Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs.
• Strong communication skills for collaborating with healthcare teams, residents' families, and social workers.
• Commitment to maintaining HIPAA compliance and respecting residents' privacy at all times.
• Flexibility to work various shifts including evenings, weekends, or overnight as needed in long-term care settings.
