Duties
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote independence and dignity.
- Monitor patient health by observing vital signs and overall condition; report any concerns promptly to healthcare professionals.
- Support medication administration following prescribed protocols while adhering to HIPAA privacy standards.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Maintain a clean and safe environment through regular cleaning duties and infection control practices.
- Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfer and mobility assistance.
- Document care plans, patient observations, and incident reports accurately in compliance with healthcare regulations.
- Provide emotional support and companionship, fostering social engagement and mental well-being for residents or clients.
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or working with individuals with developmental disabilities or dementia.
- Knowledge of patient observation techniques, medication administration, vital signs monitoring, and emergency response procedures including CPR and First Aid certification.
- Familiarity with specialized care such as Alzheimer's care, memory care, hospice care, autism support, and working with people with disabilities.
- Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs.
- Strong communication skills for collaborating with healthcare teams, residents' families, and social workers.
- Commitment to maintaining HIPAA compliance and respecting residents' privacy at all times.
- Flexibility to work various shifts including evenings, weekends, or overnight as needed in long-term care settings.
