Lenawee Community Mental Health Authority of Adrian MI is looking to hire a full-time Case Manager (for our I/DD Services group). This social work or human services position working with adults diagnosed with developmental disabilities starts at $1,038.00 per week.

A Day in the Life of a Case Manager

As a Case Manager, you assist individuals with developmental disabilities in navigating systems to better meet their needs. You understand that traditional services often aren't effective for this population and look for opportunities to increase access to additional supports. You find the gaps in support and help bring about improvement as you connect with multiple community partners and systems. Often, by the time you get involved, other systems and supports have given up or are close to it. Your job is to instill hope and reenergize efforts for recovery.

You assist consumers in identifying supports, building a team and designing a strength-based plan that focusses on recovery. The plan of service that you prepare is based upon discoveries about strengths and culture, a needs assessment and a crisis and safety support plan. You will assist the support team in developing a plan that is specific to the needs and desires of the consumer, from their perspective. You link and coordinate services and inter-agency resources.

You monitor the progress that providers are making toward outcomes and facilitate the development of transition strategies. You will find creating a path to resilience, recovery, wellness, and self-determination for the youth that you work with to be very rewarding!

Qualifications For a Case Manager

  • Bachelor's degree in a human services field from an accredited program;
  • Licensed by the State of Michigan to practice in a human services field (if applicable);
  • Education, training and/or work experience in the behavioral health field with individuals with developmental disabilities;
  • Basic computer knowledge (such as familiarity with email and internet use) and have experience with Microsoft Office applications (Word, Excel, Power Point, etc.);
  • Valid driver's license.

Work Schedule

This is a full-time position of 37-1/2-hour work weeks. The typical schedule is Monday - Friday, 8:30 AM - 5:00 PM with a one-hour unpaid lunch. There may be occasional times when he/she would need to meet with a client outside of these hours, but he/she would be offered a flexible work schedule to make up for these types of after-hour work.

The starting salary is $53,976. The annualized salary range for this position is $53,976 to $69,810 (each based on a 37-1/2-hour work week); the range is based on the number of years of service with the agency.

The Benefits Package Includes

Employees receive a generous paid leave benefit, and paid leave begins to accrue on an employee's first day of employment; an employee would receive about 24-1/2 paid days off in the first year. The number of paid leave hours an employee accrues is based on length of service; the paid leave time accruing increases after one year, five years, and ten years of employment.

Employees receive scheduled annual salary increases, starting after one year of employment.

Paid Holidays -- LCMHA observes 15 paid holidays per year; employees are eligible for paid holidays as of the first day of employment.

Employees are eligible to enroll in comprehensive medical, dental, and vision insurance as of the first day of employment.

  • Medical insurance: The employee premium contribution for the plan choices is dependent on the plan and coverage-level choice.
  • Vision insurance: the agency covers 100% of the cost for all coverage levels;
  • Dental insurance: the agency covers 100% of the cost for all coverage levels.

Employees who decline to enroll in the agency's medical insurance program because they have medical insurance coverage from a different source receive a medical insurance opt-out payment (this benefit starts as of the first day of employment).

Retirement Benefits: this benefit starts as of the first day of employment.

Long-Term Disability and Life Insurance (at no cost): the agency covers 100% of the cost; benefits start as of the 91st day of employment.

LCMHA is also a participating / qualifying site for several student debt relief options including the HRSA Loan Repayment Program & MDHHS Behavioral Health Loan Repayment Program (Please note that employee eligibility is determined by these programs, and not by LCMHA).
