Position Summary:
Lehigh Human Support Services is seeking a Part time/ Per Diem Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide professional nursing services in our Chapter 6400 Licensed Residential CLA Homes. This role ensures compliance with ODP (Office of Developmental Programs) regulations while supporting individuals with intellectual and developmental disabilities.
The nurse will be required to conduct monthly wellness checks during the first week of each month, including taking vital signs, documenting findings in the HRST system, and completing any required HRST clinical reviews. Additionally, the nurse will perform Medication Administration Record (MAR) checks and medication audits to ensure compliance and accuracy.
Key Responsibilities:
- Conduct monthly wellness checks during the first week of each month, including a full set of vitals and HRST updates.
- Perform HRST clinical reviews and ensure all health risk assessments are updated and accurately documented.
- Oversee medication administration, including MAR checks and medication audits to ensure compliance with Chapter 6400 regulations.
- Document health records, medication administration, and progress notes using Therap, ensuring compliance with HIPAA and ODP standards.
- Provide training to direct support professionals (DSPs) on medication administration, basic health care procedures, and emergency protocols.
- Monitor and coordinate medical appointments, physician orders, and follow-ups.
- Assist in developing individualized health care plans for residents in compliance with ODP guidelines.
- Maintain accurate and timely medical records, progress notes, and documentation.
- Provide emergency medical support as needed and report critical incidents following established procedures.
- Ensure compliance with all state licensing requirements and health standards.
- Communicate with physicians, case managers, and families regarding medical concerns and care plans.
Qualifications & Requirements:
- Must possess a valid Pennsylvania LPN or RN license.
- Experience working with individuals with intellectual and developmental disabilities preferred.
- Knowledge of ODP regulations, Chapter 6400 requirements, HRST assessments, and Therap documentation system is a plus.
- Strong documentation skills and ability to maintain medical records in compliance with HIPAA.
- Ability to train and educate staff on medical and health-related procedures.
- Excellent communication and organizational skills.
- Ability to work independently and manage time efficiently.
- CPR & First Aid Certification (or willingness to obtain).
Schedule & Compensation:
- Part-time position: 16-20 hours per month (must be available during the first week of each month for scheduled wellness checks).
