This candidate is responsible for accurately processing all medical record requests in a timely and efficient manner. Associate must at all times safeguard and protect the patient’s right to privacy by ensuring that only authorized individuals have access to the patient’s medical information and that all releases of information are in compliance with the request, authorization, company policy and HIPAA regulations.
DUTIES:
- Receive and process requests for patient health information and maintain electronic medical records and/or in-house charts according to company and facility policy.
- Maintain confidentiality and safeguard patient records, ensuring consistent application of medical privacy regulations and HIPAA standards.
- Ensure medical records are accurate, complete, and assembled in standard order.
- Ensure all orders or paper forms requiring a physician’s signature are signed, dated, and scanned into EMR.
- Scan and attach to pertinent resident file any paper documents such as lab results, x-rays, consultant reports, etc.
- Maintain working knowledge of facility software. Maintain medical record room in a neat and orderly fashion. Ensure all equipment is in working order.
- Arrange for medical records destruction for paper records which no longer need to be maintained.
- Notify administration of any record request. Copy, print or download records as requested.
- Follow established fire, disaster, safety, infection control and evacuation policies and procedures.
- Perform other related duties as directed by the Dean of Nursing and Administrator.
REQUIREMENTS:
- High School Diploma or GED.
- Must be 18 years of age or older.
- Basic computer proficiency.
- Strong data entry skills.
- Knowledge of ODH and OBRA regulations relevant to medical records.
- Detail and quality oriented as it relates to accurate and complete medical records information.
- Must be able to work with minimum supervision, responding to changing priorities and needs.
- Good organizational and planning skills.
- A pleasant demeanor, with the ability to interact with residents, families, and staff in a professional and cooperative manner.
- Must have compassion, tolerance, and understanding for the elderly.
Preferred:
- PCC experience.
- Experience in a health care environment.
- Experience or training in the processing of medical records.
- Knowledge of ODH and OBRA regulations relevant to medical records (preferred).
