Your Role in the Legacy Mission
As the Assistant RSD, you help ensure our community remains a place where safety, comfort, and compassion thrive. Under the direction of the Resident Services Director, you will assist with assessments, care planning, staffing coordination, and day-to-day clinical operations. What You’ll Do
Resident Care & Assessments

Assist with routine wellness checks and help address residents’ health-related questions.

Support the RSD in completing assessments prior to move-in, upon condition changes, and as required by policy or regulation.

Review and organize medical documents to ensure care needs can be met safely and appropriately.
Care Planning & Documentation

Help develop, update, and maintain individualized service plans.

Support accurate, timely documentation of health records, assessments, and wellness notes.

Assist in maintaining confidential and compliant resident wellness files.
Medication Management Support

Assist with medication ordering, scheduling, and documentation oversight.

Help train and support care staff in safe medication practices.
Family & Provider Communication

Maintain positive, professional communication with residents, families, physicians, and healthcare partners.

Assist families with questions, concerns, and care coordination needs with empathy and professionalism.
Operations & Team Support

Provide leadership and guidance to the care team, ensuring daily tasks and wellness standards are met.

Assist with scheduling, training, and supporting caregiving staff.

Participate in the on-call rotation for medical questions and resident emergencies as assigned.

Support infection control protocols and wellness initiatives.

Coordinate with the Food Service Director regarding residents’ dietary needs.

Who You Are

A compassionate, organized, detail-oriented professional committed to excellence in senior care

Licensed nurse (LPN or RN)

Experience in assisted living, senior care, or healthcare management

Skilled in assessments, care planning, and medication management

Strong leadership, communication, and critical-thinking abilities

Able to manage multiple priorities with empathy and efficiency

Able to pass background checks and meet all state requirements
