What are the responsibilities and job description for the Team Lead position at Legacy Residential Services LLC?
Position Summary:
Full-Time Team Lead is responsible for providing leadership, management and oversight for the safety and well-being of individuals with ID/A, mental health, and behavioral health needs within their assigned residential home, as required under chapter 6100/6400. The Team Lead role is to enhance the individual’s quality of life, coordinate activities and community integration in accordance with the individual’s Individual Support Plan (ISP) and lead a team of Direct Support Professionals (DSPs) to do the same.
Primary Role & Responsibilities:
Consistent with Community Homes for individuals with intellectual Disability Regulations, Chapter 6100/6400, the role of the Team Lead is as follows:
- Covers the first weekday shift (7am – 3:00pm or 8am – 4pm), Monday thru Friday.
- Manages and leads residential staff by reviewing daily logs, communicating company goals, health and safety practices, meeting deadlines and following ODP regulations.
- Completes staffing schedules in Deputy on monthly basis, makes certain all shifts are covered and covers shifts as needed.
- Makes certain shift duties, daily T-logs, GERs, fire drills, temperature checks, goal trackers and monthly Site Inspection checklists are completed.
- Manages medical appointments/outings and makes certain everything is entered on TeamUP and attended.
- Provides timely transportation to medical appointments and social outings and monitors company vehicle log, timely service and maintenance of vehicle.
- Assists with daily living requirements, including but not limited to meal prep, medication administration, housekeeping and personal care.
- Manages and reconciles monthly household expenses, i.e., food items, supplies, gas, etc.
- Retrieves individual’s mail and responds to requests for information essential to maintaining Medicaid, and SNAP benefits.
- Monitors significant social and behavioral challenges in a supportive, non-restrictive manner and adheres to approved RPPs.
- Participates in meetings with support services including BSC, SCs, family members, etc.
- Attends weekly meetings with Residential Team Lead Supervisor to review program needs, progress and needed training needs.
- Ensures reportable incidents are documented and Point Person(s) are notified immediately upon discovery.
- Completes and delivers annual performance appraisals for site DSPs.

Qualifications:
- Associates / bachelor’s degree (Preferred Related Field)
- High School/GED (Required)
- Two to Three years of residential experience (Direct Care)
- Knowledgeable with 6100/6400 regulations
- Working knowledge of Microsoft 365, Therap, and ability to utilize other forms of technology.
- Valid Driver’s License
- 21 Years of age
