???? What You’ll DoThe Resident Care Coordinator oversees the delivery of daily care in both assisted living and memory care settings, ensuring every care plan is executed with precision and compassion.
Key Responsibilities:
- Supervise, coach, and support caregivers building a strong, capable, and confident care team.
- Conduct daily rounds, review shift reports, and monitor performance on the floor.
- Oversee completion and accuracy of documentation within the ALIS platform.
- Observe residents to identify care needs, comfort levels, and satisfaction.
- Communicate promptly with the Executive Director and IDT about changes in condition.
- Ensure all infection control and safety protocols are consistently followed.
- Participate in new hire onboarding, in-service training, and ongoing staff education.
- Review and adjust caregiver assignments to maintain quality and continuity of care.
- Investigate incidents, complete reports, and support corrective action plans.
- Collaborate with the interdisciplinary team to ensure care plans remain person-centered.
- Minimum age 18 years; High School Diploma or GED required.
- Previous caregiving or medication technician experience preferred.
- Demonstrated ability to lead and train team members effectively.
- Excellent communication, time management, and organizational skills.
- Strong understanding of Title 22 and RCFE care standards (training provided).
- Ability to document accurately and maintain confidentiality.
- Physical ability to assist residents, lift up to 40 lbs, and remain active during shifts.
- CPR & First Aid Certification (current)
- TB Clearance per state regulations
- LIC 508 Criminal Record Statement
- LIC 503 Health Screening Report
- Fingerprint & Background Clearance
