This is a full-time hybrid role for a Hospice Consultant based in Overland Park, KS, with some flexibility for remote work. The Hospice Consultant will be responsible for managing relationships with patients, families, and healthcare providers to support and coordinate hospice care services. Responsibilities include meeting with referral sources, educating stakeholders about hospice services, assisting patients and families in understanding care options, and ensuring seamless transitions to hospice care. The role also involves contributing to community outreach initiatives and maintaining accurate records of activities and outcomes.
- Strong communication and interpersonal skills to effectively engage with patients, families, and healthcare professionals.
- Healthcare experience, specifically knowledge of hospice and palliative care services, regulations, and best practices.
- Organizational and time-management skills to handle multiple tasks and coordinate care plans efficiently.
- Proficiency in maintaining detailed records and reports to ensure compliance and enhance service quality.
- Ability to work both independently with minimal supervision and collaboratively as part of a team.
- Familiarity with the Overland Park, KS, healthcare community and local resources is a plus.
- Bachelor’s degree in healthcare, social work, or a related field is preferred, or equivalent work experience.
