We are looking for a skilled and compassionate
Registered Nurse (RN)
to join our
Home Health and Hospice
team. As a field RN, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homespromoting comfort, dignity, and independence.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing services including wound care, IV therapy, symptom management, and medication administration
- Monitor and document changes in patient condition and coordinate care with physicians and interdisciplinary team members
- Support patients and families with education, resources, and emotional guidance throughout the care process
- Participate in the on-call rotation, including evenings, weekends, and holidays as needed
- Ensure timely, accurate documentation in compliance with agency policies and regulatory standards
- Current RN license in the State of Texas
- Current CPR certification (BLS required)
- Minimum of 1 year of clinical nursing experience; home health or hospice experience strongly preferred
- Bilingual (English/Spanish)
- Strong clinical judgment, time management, and communication skills
- Reliable transportation, valid driver's license, and automobile insurance (required for field-based visits)
