What are the responsibilities and job description for the Patient Advocate & Care Coordinator position at Legacy Home Health Agency Inc and Careers?
Are you a "people person" with a heart for seniors and a knack for navigating the healthcare system? We are looking for a dynamic Patient Advocate to be the face of our agency. You won’t just be “selling” home care; you’ll be an essential resource, helping families access the services, equipment, and benefits they desperately need to stay safe at home.
What You’ll Do:
- Conduct Home Assessments: Visit potential clients to assess their needs for Personal Attendant Services (PAS) and determine eligibility.
- Be a Problem Solver: Identify when a patient needs more than just a caregiver—spotting needs for Home Health, Hospice, DME (walkers/hospital beds), or Meals on Wheels.
- Navigate the System: Assist families with Texas Medicaid applications and renewals via the Your Texas Benefits portal.
- Convert Leads: Turn inquiries into active clients by building trust and demonstrating how our agency improves their quality of life.
- Build a Network: Partner with local doctors, discharge planners, and DME providers to create a referral ecosystem.
What We’re Looking For:
- Experience: 1–2 years in Home Care, Social Work, or Healthcare Marketing (Texas HCSSA experience is a huge plus!)
- Knowledge: Familiarity with HHSC Form 2060, Medicaid (PHC/CAS programs), and Medicare basics.
- The "Vibe": High empathy, professional appearance, and a "go-getter" attitude.
- Mobility: Must have a reliable vehicle and a valid TX Driver’s License for daily home visits.
- Bilingual: English/Spanish required.
