Legacy Home Care, is dedicated to those we serve and to our team who make it all happen. Our services include personal care assistance, meal planning and preparation, medication management, housekeeping duties and other tasks that allow our clients to remain in their homes.
We are seeking a Care Manager that has a passion for working with seniors and is dedicated to making a difference. The ideal applicant has experience in caregiving and in managing others in a home care setting or facility that provides services to seniors. This is a full-time position with rotating on call and weekend shifts.

Qualifications
- Ability to utilize Microsoft Outlook, Word, Excel, required
- Home care experience, caregiver or care manager, required
- Proven leadership abilities
- Scheduling software, ClearCare/Wellsky preferred
- Valid drivers license, insurance and reliable vehicle, required
- Ability to pass a background check and drug screen
Responsibilities
- Schedule and participate in client and caregiver meetings
- Schedule shifts by matching the needs of our clients with caregiver skills, experience, availability and personality
- Continually track client and employee satisfaction to resolve any complaints that arise
- Work under direction of Director of Client Services to ensure caregiver and client satisfaction
- Meet with clients and families to assess care needs
- Introducing caregivers to clients
- Conduct regular home visits
- May need to fill in for caregiver shifts at client homes
- Participate in after hours and weekend on call rotation
Salary : $21 - $24
