Avantara Saint Cloud is a fully licensed Memory Care facility specializing in Dementia, Alzheimer's and other memory care needs. On average, our population is admitted for long term care needs with a skilled census average of 5-7 residents. Our facility is licensed for 78 memory care beds and our average census is between 70-75 residents.
As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident's unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara St. Cloud by developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
To be eligible for consideration applicants should have:
As a minimum, an unencumbered State of South Dakota R.N. License OR LPN License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. or LPN in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
You will have great resources with the company and facility in house staff to provide training as you learn this new role and how to successfully, accurately, and thoroughly complete an MDS Assessment per the RAC Manual.
