As a Behavioral Health Clinician (BHC), you will work in a consultative and collaborative role as an integral member of the primary care team, providing behavioral health interventions that support the overall health and well-being of patients. In this role, you will help address a broad spectrum of behavioral health needs within the primary care population by identifying concerns early and providing problem-focused, evidence-based interventions that promote wellness and relieve symptoms. Responsibilities
- This position requires advanced level skill in delivering evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems.
- The work includes the BHC meeting with patients individually for an assessment and then providing recommendations to the primary care team to enhance the patient's care plan.
- When indicated, the BHC may also work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning.
- The BHC also supports helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.
Education:
- Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.
- Minimum two years of clinical treatment experience with patients and working as part of a multidisciplinary care team.
- Ability to diagnose and treat mental health and substance use disorders for the identified clinic population.
- Competent to deliver evidenced based therapies such as cognitive behavioral therapy, acceptance and commitment therapy, short term psychodynamic psychotherapy, dialectal behavioral therapy, trauma focused therapy and solution focused interventions.
- Ability to collaborate and advocate for patients’ behavioral and physical health needs with the referring provider.
- Ability to engage patients in health behavior change through brief interventions and health/psychoeducation .
- Coordinate with referral resources and other clinic providers and, where appropriate, community partners to facilitate referral to appropriate specialty treatment.
- Highly adept social skills, with strong professional presence.
- Demonstrated competencies in diagnostic assessment of emotional, cognitive, behavioral and substance use disorders for population(s) served in clinical settings.
- Demonstrated competency in assessment of cognitive and psychological problems associated with and/or exacerbating medical conditions.
- Experience with delivering trauma informed care.
- Experience with individual psycho/social treatment and conducting groups.
- Experience treating patients with acute and chronic physical health conditions .
- Licensed in one of the following within applicable state:
- Clinical Social Worker (LCSW)
- Licensed Independent Clinical Social Worker (LICSW)
- Licensed Professional Counselor (LPC)
- Licensed Mental Health Counselor (LMHC)
- Licensed Marriage and Family Therapist (LMFT)
