Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer devices.
- Monitor and record patient vital signs, including blood pressure, pulse, temperature, and respiration, ensuring accurate medical documentation in Electronic Health Record (EHR) systems.
- Observe and report changes in patient condition promptly to nursing staff, maintaining detailed medical records while respecting HIPAA privacy standards.
- Support patients with medication reminders and basic medication administration under supervision, following prescribed protocols.
- Provide compassionate Alzheimer's care and dementia support by implementing specialized memory care techniques tailored to individual needs.
- Assist with catheter care, wound care, and other medical procedures as directed by nursing professionals.
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning patient areas, and meal preparation aligned with dietary restrictions.
- Engage in home & community care activities that promote comfort and well-being within the patient's living environment.
- Demonstrate proficiency in medical terminology relevant to hospice care and participate in ongoing training for CPR and other certifications required for patient safety.
Experience
- Prior experience in assisted living facilities or nursing homes is highly preferred.
- Demonstrated ability working with people with developmental disabilities or disabilities of any kind enhances your caregiving approach.
- Experience in home health or long-term care settings provides a strong foundation for this role.
- Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems is advantageous for maintaining accurate documentation.
- Certification in CPR is required; additional certifications such as hospice care training or Alzheimer's/dementia care are a plus.
