Under general supervision performs a wide range of tasks in the community to assist clients in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to clients and their families. Monitor client progress towards treatment goals. Extensive documentation required; accurately charts services and client progress. Must be able to work well with community agencies and function as part of a team..
The Case Manager will be supervised by the Clinical Team Leader.
Requirements:
Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.
Knowledge of mental illness
Valid GA driver’s license with less than 5 points

Working Conditions: Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations. May have extended periods of sitting, walking, and standing. Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in BHSGA or personal vehicle.
