Key Responsibilities:
- Develop and manage relationships with healthcare professionals, clinics, hospitals, and other medical facilities.
- Evalutate prospective patients prior to admission.
- Complete clinical assessments and determine appropriateness for admission.
- Prepare and complete required pre-admission screening documentation.
- Excellent communication skills—both verbal and written—to effectively. communicate with referral sources, patients, families, and the treatment team.
Qualifications:
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licenses required
- Strong clinical assessment and communication skills
- Excellent organization and documentation skills
- Ability to work collaboratively with patients, families, referral sources, and healthcare professionals
Pay: From $55,000.00 per year
