License/Certification Requirements: Valid RN or PHN State Nursing Licensing/Certification Required; Minnesota State Licensing/Certification preferred; CPR Certification and maintenance. Valid MN Drivers License, current insurance or be insurable.

Summary: Works with women (prenatal and post partum) at the highest risk for having poor birth outcomes; providing home visits, acting as a referral source for clients, making physical assessments, screening for risk factors and setting goals for the client with follow-up made through the first three years of the child's life.

Education Requirements and Experience:

Licensed Registered Nursing or Public Health Nursing Certification is required.

Prefer experience in Public Health but are willing to train for position.

Additional Qualifications and Skills:

Excellent time management, organizational, conflict resolution, public relations and speaking skills are required.

Ability to establish and maintain effective working relationship with staff, clients and other professionals.

Ability to work within the team concept and accept supervision is required.

Ability to effectively and efficiently communicate, both verbally and in writing is required.

All licensing renewal requirements as specified by the MN State Board of Nursing or as required by the state they are licensed in, including required CEUs.

Familiarity with the Leech Lake Reservation and surrounding communities is preferred.

Experience in utilization of EHR technology is required.

Native American preference.

Must meet Health Division hiring guidelines.

Pre-employment and random drug testing is mandatory for all Leech Lake Band of Ojibwe positions.

Supervisory Duties:

None

Duties/Responsibilities:

Provide high quality health care services and ensure accessibility and availability of health services.

Provides services for identified prenatal population of the Leech Lake Reservation.

Make contact with mothers at high risk for poor birth outcomes through prenatal clinics, referral sources & community services.

Makes physical assessments; screening for risk factors; case management, smoking status assessment, housing assessment, domestic violence assessment, depression screening and substance abuse assessment.

Develop Case management plans with client input to achieve the best pregnancy outcome.

Responsible for maintaining accreditation, CEU's and renewal fees, and must submit to supervisor upon each completion.

Working collaboratively with appropriate entities and other agencies to ensure proper outcomes of client and/or family (Social, behavioral, legal, and other public health entities).

Required to have 50-75% of the work week meeting with clientele; willing to go into client homes.

Organize and teach childbirth education classes.

Responsible for monthly reporting to supervisor as it relates to MCH Nursing.

Work with WIC as needed.

Provide community educations & trainings: Group Sexual Education and prevention classes.

Teen pregnancy prevention activities and education.

Work closely with the Advocates and other LLBO departments to reinforce positive health behaviors (good nutrition, smoking cessation, breastfeeding, exercise, diabetes control, etc.).

Educate and effectively communicate with those served concerning the high risk pregnancies and appropriate preventive measures and use of the health care system.

Appropriate, accurate and complete medical records entries through EHR system.

Work closely with the MCH Coordinator and other nurses in program development.

Participate in local meetings, consortiums and action teams when requested.

Attend and or conduct all mandatory meetings and trainings as required.

Maintain confidentiality and professional ethics at all times.

Travel in and out of the state on a possibly regular basis.

Understanding and sensitivity to Native American culture.

Adhere to policies governing staff conduct.

All other related duties as assigned by Director of Nursing.

Physical Requirements:

May be exposed to communicable diseases and/or blood borne pathogens.

May require driving in sub-optimal weather and road conditions.

May require long periods of sitting at a desk or computer.

May be subject to secondhand smoke as a result of home visits.

May require occasional modification of workstations (this position will need to provide services in a large variety of settings and community locations).



Compensation: DOQ
Additional Details : Monday - Friday, some evenings and weekends
8:00am to 4:30pm

Salary : $87,536 - $119,119
