We are searching for a Nurse Case Manager for our program located in Raleigh, NC.
Essential duties and functions
- Responsible for all case provisions for the active rehabilitation caseload, for both the residential neurorehabilitation and day treatment clients
- Leads team meetings and interfaces with the treatment team to establish rehabilitation goals and coordinates treatment necessary to support the client’s treatment plan
- Responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up
- In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team
- Serves as client advocate by communicating clients’ needs and issues to team members and other designated individuals, including a plan for resolution and follow-through as indicated
- Support the Program Director in program case managers’, onboard training, and performance to ensure case management standards of performance meet expectations
Requirements
- State licensure as a Registered Nurse is preferred
- A bachelor's degree along with case management and health care industry experience is required
- A master's degree and CRC or CCM designation is preferred
- Must have a valid driver's license and maintain responsible driving behaviors
