Position Overview

We are seeking a compassionate and dependable Hospice Certified Nursing Assistant (CNA) to join our team on a PRN basis. The Hospice CNA provides hands-on personal care and support to hospice patients in their homes while working under the direction and supervision of the hospice nursing team.

This position is ideal for a CNA who values meaningful one-on-one patient care and wants the flexibility of a PRN schedule while making a difference in the lives of patients and families.

Key Responsibilities

• Provide compassionate personal care to hospice patients in the home setting.
• Assist with bathing, grooming, dressing, toileting, hygiene, and other activities of daily living.
• Assist patients with mobility, positioning, transfers, and maintaining a safe environment.
• Follow the patient's established plan of care and instructions from the hospice nurse.
• Observe and report changes in the patient's condition to the appropriate nurse or supervisor.
• Provide companionship, emotional support, and reassurance to patients and families.
• Assist with light household tasks related to the patient's care and comfort.
• Maintain accurate documentation of services provided.
• Communicate professionally and respectfully with patients, families, caregivers, and members of the hospice team.
• Maintain patient confidentiality and follow all hospice, HIPAA, safety, and infection-control requirements.

Qualifications

Required:

• High school diploma or GED.
• Current CNA certification in good standing in the State of New Mexico.
• Completion of an approved CNA training and competency program.
• Valid driver's license and current automobile liability insurance.
• Reliable transportation.
• Ability to work independently while following the direction of the hospice nursing team.
• Ability to pass required background screening.

Preferred:

• Previous CNA, home care, hospice, or home health experience.
• Experience caring for elderly or medically fragile patients.
• Current CPR/BLS certification.
