The role
- Manage and deliver patient care plans in a home setting
- Work independently while supported by a clinical team
- Coordinate care and complete documentation

Schedule
- Weekend-focused role:
- Saturday & Sunday patient visits
- On-call Friday, Saturday, Sunday evenings (5pm–10pm)
- 2 holidays per year
What they’re looking for
- Active RN license (PA)
- 1 year RN experience
- 1 year of clinical documentation, clinical informatics, OR clinical quality
- 1 year of EMR, Other EMR, OR practice partner EMR
- Comfortable working independently and managing your own schedule
- Experience with EMR/documentation systems
- Associate degree or higher
- Okay with the required local travel
Certifications:
- AHA ACLS or ARC ALS
Compensation
- Estimated earnings: $77K–$100K (based on productivity)
- Per visit: $42–$46
- $10K sign-on bonus
- Quarterly bonuses
