What are the responsibilities and job description for the Outreach APRN position at Lawrence Douglas County Health Department?
Provide preventive care, health screenings, physical exams, laboratory testing, and immunizations in accordance with Lawrence-Douglas County Public Health (LDCPH) protocols.
Perform and interpret point-of-care and diagnostic laboratory tests, and communicate results to clients.
Conduct comprehensive patient assessments, including medical histories, diagnostic testing, and treatment planning for acute and chronic conditions.
Provide reproductive health services, including contraception counseling, STI testing and treatment, pregnancy testing, and basic gynecologic care.
Offer education and counseling on family planning options, contraceptive methods, and preconception health to support patients’ reproductive choices and overall wellness.
Develop and implement patient care plans based on evidence-based practices and patient goals.
Initiate, adjust, and monitor low-level psychotropic medications, such as for mild depression or anxiety, in collaboration with behavioral health specialists and within scope of practice.
Provide accurate, evidence-based education and counseling on disease prevention, medication adherence, and self-management of chronic conditions.
Promote a holistic approach to health by integrating physical and mental well-being into all treatment plans.
Deliver education and counseling on topics such as wellness, chronic disease prevention, nutrition, safe sex practices, and other public health issues to reduce health risks and improve community well-being.
Provide resources and guidance to patients and families for managing both physical and mental health conditions.
Proactively deliver care in dynamic environments, including patient homes, encampments, and community settings in collaboration with Lawrence-Douglas County Fire Medical’s Mobile Integrated Health (MIH) team and the City of Lawrence Homeless Response Team (HRT).
Participate with MIH and HRT partners in client tracking, follow-up, referral, and reporting to ensure coordinated, continuous care for high-need populations.
Coordinate care across systems and refer patients to appropriate services, acting as a client advocate to support access and continuity.
Maintain accurate, timely, and confidential documentation in electronic health records.
Consult with the medical consultant as appropriate to ensure quality and safety of care.
Ensure compliance with all local, state, and federal regulations related to patient care, safety, and medication management.
