The Behavioral Health Care Coordinator is responsible for clinical and administrative support tasks related to facilitating behavioral health and psychiatric care at Lawndale Christian Health Center. Responsibilities include coordination of clinic and telehealth Behavioral Health consultation visits, project management of initiatives related to behavioral health and psychiatry, development of clinical resources for patients and providers, and data collection and analysis of service line utilization and quality outcomes.

The Behavioral Health Care Coordinator is responsible for clinical and administrative support tasks related to facilitating behavioral health and psychiatric care at Lawndale Christian Health Center. Responsibilities include coordination of clinic and telehealth Behavioral Health consultation visits, project management of initiatives related to behavioral health and psychiatry, development of clinical resources for patients and providers, and data collection and analysis of service line utilization and quality outcomes.
Principal Duties and Responsibilities *
- Coordinate with care teams to facilitate on-site BH handoff process, including following up with care teams members regarding workflow concerns, updating materials and signage in consult rooms and provider areas, and assisting with orientation of new providers and MAs to use of BH services, schedules, and telehealth equipment.
- Facilitate BH telehealth visits, as needed, including coordination of triage schedule, confirming patient availability/interest for scheduled visit, and administration of secondary diagnostic assessments (i.e., PCL-5, DAST, MDQ, ASRS, etc.), and.
- Provide clinical and technical support to Behavioral Health treatment groups (i.e., THRIVE Teen Group, Stress Management Group, etc.) as needed, including curriculum development, co-facilitation, patient outreach, registration, training of BH staff on telehealth platform, troubleshooting technical concerns, coordinating communication with care teams regarding groups (i.e., schedule updates, flyers/signage, EMR quick text, outreach to Site Managers and PCPs, etc.).
- Assist in the development of EMR form content to improve efficiency and quality of documentation for BH, psychiatry, and medical providers.
- Assist in the development and updating of provider EMR schedules and templates, as needed.
- Develop, organize, and maintain clinical resources for patients and care team members related to mental health concerns (i.e. patient education handouts, condition-specific referral guides, clinical decision support tools, etc.)
- Compile items for intra- and interdepartmental communication in monthly departmental newsletter.
- Coordinate internal pediatric psychiatry consultation requests, gather data related to case review, meet weekly with psychiatrist to facilitate case review.
- Assist with planning and coordination of events, meetings, and trainings for BH department and external partners, including scheduling, purchasing supplies, preparing agendas, and capturing meeting notes.
- Run productivity reports monthly to highlight areas for operational improvement.
- Collect and report program-specific data used to monitor clinical quality and work toward achieving CQI program goals.
- Perform other duties as required to help LCHC accomplish its mission.

- Bachelor’s Degree or equivalent preferred.
- Advanced computer skills necessary; experience with reporting tools and/or within EMR preferred.
- Basic knowledge of common mental health and substance abuse disorders, including psychopharmacology and consultation models within integrated care; ability to learn to support ongoing program development and administration
