Position Summary
The Mental Health Assessor/Counselor is responsible for conducting comprehensive mental health assessments (initial and annual), including DSM diagnoses and collaborative treatment planning. This role also provides individual counseling to adults with a primary diagnosis of Severe Mental Illness (SMI).

The Mental Health Assessor/Counselor is responsible for conducting comprehensive mental health assessments (initial and annual), including DSM diagnoses and collaborative treatment planning. This role also provides individual counseling to adults with a primary diagnosis of Severe Mental Illness (SMI).
Key Responsibilities
- Complete assessments and treatment plans in compliance with Medicaid, OARs, and agency policy
- Provide individual counseling and psychoeducation, including in-home or community-based services
- Deliver crisis intervention and face-to-face assessments as needed
- Collaborate with internal teams, community partners, and natural supports
- Support transitions of care and attend team/supervisory meetings
- Maintain professional, ethical standards and meet productivity expectations
- Document all services in the electronic health record system
Required
- Must meet QMHP certification requirements at time of hire
- Minimum 1 year experience working with individuals with SMI
- Valid Oregon driver's license
- Successful background check, fingerprinting, and drug screening
- Experience with co-occurring disorders
- Ability to assess, diagnose, and provide evidence-based interventions
- Holds a current Oregon license (LCSW, LPC, LMFT)
- Meets all state licensure requirements
- Licensed QMHP: $75,000 β $85,000/year
- Salary based on experience and qualifications. Full-time, exempt position with benefits.
