We are currently seeking Service Coordinators to join our busy service coordination team who are the first point of contact for our clients.
The Service Coordinator will be responsible for organising aspects of the Support at Home services for our clients, based on assessments completed by the Care Partners. This role involves liaising with clients, families, providers and other healthcare professionals to ensure that services are delivered efficiently and effectively. Your primary focus will be on providing excellent customer service and maintaining accurate records.
