- Plans and organizes patient care according to protocols and pathways.
- Collaborates with the healthcare team to develop and implement treatment plans.
- Conducts health assessments and orders necessary tests.
- Prescribes medications and medical equipment as per practice protocols.
- Evaluates treatment effectiveness and modifies plans as needed.
- Promotes professional growth and serves as a clinical resource.
- Assists Clinical Access Center staff when necessary.
- Completes additional tasks as directed by supervisor
- License: Current registration as Nurse Practitioner in New York State
- Education: Master's degree in nursing
- Certification as an Adult or Family Nurse Practitioner
