Position Overview
LaSante Health Center is seeking a compassionate and organized Housing Specialist to support patients experiencing housing instability, eviction risk, homelessness, rental arrears, and other housing-related challenges.
Responsibilities- Assess patients’ housing needs and develop action plans.
- Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental-assistance, and eviction-prevention applications.
- Help patients gather documents and complete applications.
- Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations.
- Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
- Track applications, deadlines, and patient progress.
- Work closely with therapists, case managers, and medical providers.
- Experience in housing services, case management, social services, or a related field preferred.
- Familiarity with New York City housing resources required..
- Strong communication, organization, documentation, and follow-up skills.
- Ability to work compassionately with vulnerable individuals and families.
- High school diploma required; associate’s or bachelor’s degree preferred.
- Bilingual skills are a plus.
- $24–$28 per hour
