Position: Licensed Nurse Consultant (Contractor)
Department: Adult Residential Model (ARM)
Supervising Directors: Executive Director and Director of Residential Services
Classification: Consultant
Hours Worked: 6 Hours per week/Hours needed may vary
Consultant Fees: $90.00 per hour
At the discretion of the Executive Director, Director of Residential Services and physicians providing medical services to individuals served by the agency, the Licensed Nurse Consultant shall implement medical care within the scope of the position and license held. The Licensed Nurse Consultant shall coordinate care of individuals with developmental disabilities including, but not limited to: medication oversight, attend client-based meetings and clinical review meetings, implement routine care that cannot be provided by of a licensed community care facility per California Code of Regulations, Title 22 (Title 22), review of lab work and diagnostic testing, emergency medical visits, providing basic first aid, development of Restricted Health Condition Care Plans (RHCCPs) per California Code of Regulations Title 22, and provide with training on the implementation of RHCCPs.
This consultant position is a temporary position and is funded through contract with the Regional Center of the East Bay. Should the contract be discontinued, Las Trampas may, or may not, choose to continue the position. The consultant will be responsible for submitting accurate and timely billing to the Las Trampas Business Services Office for services rendered. Billing must be submitted indicating the number of hours spent with each client and indicate the primary cost center.
Responsibilities and Duties:

Monitor, record, and report symptoms or changes in participants' conditions in accordance with licensing regulations.

Maintain accurate, detailed reports and records.

Record participants' medical information and vital signs.

Administer medications, treatments, first aid, PRN medications, and routine care not allowed by a Community Care Facility as per Title 22 as needed.

Provide oversight of medication management at Adult Residential Model facility, as needed.

Under the direction of a physician, order, interpret, and evaluate diagnostic tests to identify and assess participants' conditions and provide consultation to program management on the care and treatment of client medical conditions.

Under the consultation of a physician, modify participant treatment plans as indicated by participants' responses and conditions.

Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate participant care plans.

Develop Restricted Health Condition Care Plans (RHCCPs) and provide necessary training to implement the plans in adherence to licensing regulations as outlined in Title 22.

Monitor all aspects of participant care, including diet and physical activity.

Instruct individuals, families, or other groups on topics such as health education and disease prevention and develop health improvement programs.

Communicate pertinent medical information per protocols to others as directed by a physician.

Consult with program management regarding the health and well-being of participants under their care.
Physical Requirements:

Ability to lift 50 lbs. and assist individuals with physical disabilities and mobility barriers to transfer from a wheelchair to either a seated or supine position

Ability to be in a seated position for periods of up to 2 hours

Visual acuity required for the use of computers and reading materials

Pass a tuberculosis (TB) screening
Qualifications:

Current and valid license from the associated nursing board registry applicable to the level of nursing provided

Current and valid proof of liability/malpractice insurance in the amount of $1,000,000

Current and valid California driver’s license as well as an operating vehicle, vehicle insurance, and driving record that meets agency requirements

A minimum of 2 years’ experience providing nursing services to individuals with developmental disabilities (may be substituted for experience with similar populations/disabling conditions)

Ability to effectively communicate information and ideas in written and verbal format and build and maintain relationships

Team player with the confidence to take the lead and provide guidance to other departments when necessary

Strong verbal and written communication skills

Ability to understand written and verbal directives

Ability to pass a criminal background clearance through the Department of Justice
