We are seeking an experienced and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our healthcare team. The ideal candidate will have a strong clinical background and specialized expertise in Medicare Advantage plans Wellmed, Cigna, Prominence, risk adjustment documentation, and RAF (Risk Adjustment Factor) scoring.
Key Responsibilities
- Conduct comprehensive clinical assessments and patient evaluations within Medicare Advantage guidelines.
- Ensure accurate, timely, and compliant documentation of all patient encounters and chronic conditions.
- Capture precise diagnostic coding to reflect patient acuity and support proper RAF scores.
- Collaborate with care teams to close care gaps and support quality and STAR measures.
Qualifications & Requirements
- Active, unencumbered state NP or PA license in Texas
- Current board certification (ANP, FNP, PA-C, etc.).
- Minimum 1–3 years of clinical experience working directly with Medicare Advantage plans.
- Demonstrated hands-on experience with RAF scoring, diagnostic coding, and risk adjustment.
- Strong electronic health record (EHR/EMR) proficiency.
