Job Summary
Laramie Health and Rehabilitation is seeking a dedicated Physical Therapist (PT) or Physical Therapy Assistant (PTA) to join our exciting and fast-paced rehabilitation team. Team members provide high-quality rehabilitative care focused on improving residents’ functional abilities, independence, and overall quality of life in a compassionate and supportive environment.
Essential Functions
Comply with State practice acts and/or American Physical Therapy Association (APTA) guidelines.
Provide physical therapy evaluations, screenings, and treatments to determine and improve residents’ functional levels.
Develop, implement, and carry out treatment plans established by the Physical Therapist with measurable, functional outcomes and physician orders.
Monitor residents’ responses to treatment and modify care plans as appropriate with physician approval.
Provide direct resident care and therapeutic interventions.
Maintain clear, concise, and timely documentation in medical records in accordance with reimbursement guidelines and facility policy.
Document and sign treatment encounters, supervision activities, and progress notes as required.
Participate in discharge planning and resident/family care meetings.
Collaborate with the Rehabilitation Manager (RM) and interdisciplinary team regarding resident care and operational concerns.
Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehabilitation services.
Maintain productivity standards established by the Rehabilitation Manager.
Participate in quality improvement and Quality Assurance Review activities.
Follow policies and procedures related to infection control, emergency preparedness, and workplace safety.
Maintain consistent and predictable service hours as assigned.
Supervise PT assistants and aides when applicable.
Review and communicate caseload information regularly with supervising therapists and staff.
Qualifications
Physical Therapist (PT)
Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
Successful completion of the National Certification Examination for Physical Therapists.
Current state licensure where applicable.
Physical Therapy Assistant (PTA)
Associate degree from an accredited Physical Therapy Assistant program.
Current state licensure where applicable.
