Responsibilities
- Assess client needs through active listening and thorough case evaluations, developing personalized service plans.
- Coordinate with social service agencies, healthcare providers, housing programs (including Section 8), Medicaid, Medicare, and workforce development initiatives to secure resources for clients.
- Provide crisis intervention and crisis management support during emergencies or urgent situations, ensuring client safety and well-being.
- Deliver addiction counseling and behavioral health support, utilizing motivational interviewing techniques to promote positive behavioral changes.
- Assist clients with working with seniors, individuals with disabilities, or those involved in child welfare or childcare programs by connecting them with appropriate services.
- Maintain accurate documentation of client interactions, service plans, and progress notes while demonstrating strong writing skills.
- Supervise and mentor junior staff or volunteers when applicable, fostering a team-oriented environment focused on client empowerment.
Skills
- Proven experience in social work or social services with a focus on case management and client communication.
- Expertise in crisis intervention, crisis management, behavioral health support, and addiction counseling.
- Strong knowledge of Medicaid, Medicare, Section 8 housing programs, child welfare policies, and working with geriatric patients.
- Ability to work effectively with culturally diverse communities and individuals with disabilities through culturally sensitive approaches.
- Bilingual skills are highly desirable to serve a broader range of clients effectively.
- Certifications in CPR and First Aid are preferred to ensure safety during emergency situations.
- Excellent organizational skills combined with the ability to multitask efficiently while maintaining attention to detail.
